- Cost is $25.
- Sports physicals by Self Regional Healthcare will be good for any sport played during the 2023-2024 school year.
- Student must be checked in and paid by 7 p.m.
- The Physical Form, found here, must be completed prior to the physical and signed by a parent or guardian.
